       目的 探讨应用超声声脉冲辐射力成像技术(ARFI)在窒息后早产儿脑损伤中的临床应用价值。方法 选取有窒息史早产儿31例(早产窒息组),对照组为早产儿30例。采用Siemens Acuson S2000超声诊断系统,使用4V1探头,于ARFI模式下启动声脉冲,测量颅脑双侧顶叶、丘脑及大脑镰声触诊组织量化功能(VTQ)值。结果 与对照组相比,早产窒息组双侧顶叶及右侧丘脑VTQ值差异有统计学意义(P<0.05);早产窒息组室管膜下出血部位VTQ值与对照组双侧顶叶VTQ值比较差异无统计学意义(P>0.05);2例早产儿脑白质软化多部位VTQ值为0。对照组颅脑顶叶与丘脑VTQ值稳定,且丘脑VTQ值大于顶叶和大脑镰(P<0.05)。结论 ARFI能实时、敏感探测早产儿脑部不同部位正常与病变VTQ值,有望成为早期诊断早产儿脑损伤的手段之一。

      Objective To assess brain injury in early prematures with asphyxia using ultrasound acoustic radiation force impulse imaging (ARFI). Methods Thirty-one prematures with asphyxia (prematures with asphyxia group) and thirty healthy prematures (control group) were enrolled in this study. ARFI (Siemens Acuson S2000 ultrasound with 4V1 transducer) was applied to measure the value of virtual touch tissue quantification (VTQ) of the bilateral parietal lobe, thalamus and the cerebral falx in every subject. Results The value of VTQ in thalamus was higher than that in the parietal lobe and cerebral falx in control group. Compared with control group, the value of VTQ in the bilateral parietal lobe and the right thalamus was higher in prematures with asphyxia group (P<0.05). The value of VTQ had no significant difference between the subependymal hemorrhage positions in prematures with asphyxia group and bilateral parietal lobes in control group (P>0.05). The value of VTQ in several spots of the two periventricular leukomalacia was zero. Conclusion ARFI can real-time and sensitively detect the value of VTQ in the different spot of prematures brain, may be a potentially useful tool to evaluate the brain injury in prematures.
