      目的 观察MR酰胺质子转移加权(APTw)成像诊断粗梁-团块型肝细胞癌(MTM-HCC)的价值。方法 前瞻性纳入56例拟接受上腹部MR检查的肝占位患者,根据病理结果分为MTM-HCC组(n=12)与非MTM-HCC组(n=44)。行上腹部MR扫描,包括常规及APTw成像;比较组间MRI表现及相关参数,评估其诊断MTM-HCC效能。结果 组间肿瘤最大径、动脉期乏血供成分占比、坏死占比、根据改良Mulé标准诊断的MTM-HCC占比及APTw值差异均有统计学意义(P均<0.05);上述参数诊断MTM-HCC的曲线下面积(AUC)分别为0.667、0.678、0.667、0.652及0.775,以APTw值的AUC最高(P均<0.05)。基于肿瘤最大径、改良Mulé标准及APTw值构建的联合模型的AUC为0.866,显著高于除APTw值外的任一单一项(P均<0.05)。结论 MR APTw成像有助于诊断MTM-HCC。

      Objective To observe the value of MR amide proton transfer-weighted (APTw) imaging for diagnosis of macrotrabecular-massive hepatocellular carcinoma (MTM-HCC). Methods Totally 56 patients with liver space-occupying lesion who would undergo upper abdominal MR examinations were prospectively enrolled and divided into MTM-HCC group (n=12) and non MTM-HCC group (n=44) based on pathology. Upper abdominal MR scanning was performed, including routine and APTw imaging. MR manifestations and parameters of tumors were compared between group, and their efficacy for diagnosing MTM-HCC was observed. Results Significant differences of tumor maximum diameter, the proportion of arterial phase avascular components, necrosis and MTM-HCC diagnosed according to the modified Mulé criteria, as well as of APTw value were found between groups (all P<0.05). The area under the curve (AUC) of the above ones for diagnosing MTM-HCC was 0.667, 0.678, 0.667, 0.652 and 0.775, respectively, and APTw value had highest AUC (all P<0.05).A combined model was constructed based on tumor maximum diameter, modified Mulé criteria and APTw value, and its AUC was 0.866, significantly higher than any single expression/parameter except APTw value (all P<0.05). Conclusion MR APTw imaging was helpful for diagnosing MTM-HCC.
