| 目的 观察血管超声联合新型血栓分子标志物预测膝关节置换术后症状性深静脉血栓(DVT)的价值。方法 前瞻性纳入258例拟接受膝关节置换术患者,根据术后7天内下肢静脉血管超声检查结果分为DVT组(n=34)与非DVT组(n=224);比较组间凝血功能指标、血常规指标、新型血栓分子标志物、血管超声参数及手术相关资料,筛选DVT的危险因素及其保护因素;构建联合模型,观察其预测价值。结果 D-二聚体、C-反应蛋白、中性粒细胞百分比、平均血小板体积/淋巴细胞比值、中性粒细胞/淋巴细胞比值、血小板/淋巴细胞比值、血栓素A2、中性粒细胞胞外诱捕网,股静脉平均血流速度、股静脉流速峰值,以及术前住院时间和手术时间均为DVT的危险因素,淋巴细胞比率及6-酮-前列腺素F1α则为其保护因素(P均<0.05);据此构建的联合模型预测DVT的曲线下面积为0.749。结论 血管超声联合新型血栓分子标志物有助于预测膝关节置换术后症状性DVT。 |

| Objective To observe the value of vascular ultrasound combined with novel thrombus molecular markers for predicting symptomatic deep vein thrombosis (DVT) after knee arthroplasty. Methods Totally 258 patients who would undergo knee arthroplasty were prospectively enrolled and divided into DVT group (n=34) and non-DVT group (n=224) based on lower limb venous ultrasound findings within 7 days postoperatively. The indicators of coagulation function, blood routine indicators, novel thrombus molecular markers, vascular ultrasound parameters and surgery relative data were compared between groups to screen the risk and protective factors of DVT. Then a combined model was established, and its predictive value was observed. Results D-dimer, C-reactive protein, neutrophil ratio, mean platelet volume-to-lymphocyte ratio, neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, thromboxane A2, neutrophil extracellular traps, the average blood flow velocity and peak flow velocity of femoral vein, as well as preoperative hospitalization time and surgical time were all risk factors, while lymphocyte ratio and 6-keto-prostaglandin F1α were both protective factors of DVT (all P<0.05). A combine model was constructed, and its area under the curve for predicting DVT was 0.749. Conclusion Vascular ultrasound combined with novel thrombus molecular markers were helpful to predicting symptomatic DVT after knee arthroplasty. |
