| 目的 观察3D导航辅助带袢钛板固定术治疗下胫腓联合(DTS)损伤的价值。方法 前瞻性将126例DTS损伤患者随机分为观察组(n=63)与对照组(n=63),分别行3D导航辅助带袢钛板固定术及传统螺钉固定术;比较组间手术相关指标、血清炎症指标、足踝功能指标、CT参数及并发症。结果 观察组手术时长、术后1周距骨-腓骨-跟骨稳定性评分和术后3、12及24个月美国足踝外科协会评分和踝关节功能评分均高于,而术后首次负重时间、骨折临床愈合时间、术后1及3个月血清炎症指标,以及术后3、12及24个月胫-腓骨中心距离、胫-腓骨间隙宽度及胫-腓骨前缘距离均低于对照组(P均<0.05)。术后观察组3例发生内固定物松动,骨折愈合延迟、切口感染及静脉血栓各1例;对照组分别为5、7、2及4例;观察组骨折愈合延迟占比低于对照组(P<0.05)。结论 3D导航辅助带袢钛板固定术治疗DTS损伤疗效佳且安全。 |

| Objective To observe the value of 3D navigation-assisted loop titanium plate fixation for treating distal tibiofibular syndesmosis (DTS) injuries. Methods Totally 126 patients with DTS injuries were prospectively enrolled and randomly divided into observation group (n=63) and control group (n=63), who then underwent 3D navigation-assisted looped titanium plate fixation and conventional screw fixation, respectively. The surgical related indicators, serum inflammation indicators, ankle function indicators, CT parameters and complications were compared between groups. Results The surgical duration, talar-fibular-calcaneal stability score 1 week after surgery, American Orthopaedic Foot & Ankle Society scores, as well as ankle joint function scores 3, 12 and 24 months after surgery in observation group were higher, while the first postoperative weight-bearing time, clinical healing time of fractures, serum inflammatory indicators 1 and 3 months after surgery, as well as tibiofibular center distance, width of tibiofibular gap and tibiofibular anterior margin distance 3, 12 and 24 months after surgery were all lower than those in control group (all P<0.05). After surgery, 3 cases of internal fixation loosening, 1 case of delayed fracture healing, 1 case of incision infection and 1 case of venous thrombosis were found in observation group, while 5, 7, 2 and 4 cases were found in control group, respectively, and observation group had lower proportion of delayed fracture healing (P<0.05). Conclusion 3D navigation-assisted loop titanium plate fixation for treating DTS injuries was effective and safe. |
