      目的 观察基于颈内静脉直径、颈总动脉直径及二者比值预测植入完全植入式静脉给药装置(TIVAD)风险的价值。方法 回顾性纳入46例接受经颈内静脉入路植入TIVAD的恶性肿瘤患者,记录其颈内静脉直径、颈总动脉直径及二者比值,以及穿刺次数、颈总动脉损伤及皮下血肿情况等;分析各参数间的相关性,评估基于颈内静脉直径、颈总动脉直径及二者比值预测穿刺次数及颈总动脉损伤的效能。结果 颈内静脉直径、颈内静脉直径/颈总动脉直径及皮下血肿均与穿刺次数呈中度相关(r=-0.613、-0.594、0.588,P均<0.05);前二者与颈总动脉损伤呈低度相关(r=0.323、-0.482,P均<0.05)而颈总动脉损伤则与皮下血肿呈高度相关(r=0.884,P<0.05)。以颈内静脉直径及颈内静脉直径/颈总动脉直径预测植入TIVAD首次穿刺成功的曲线下面积(AUC)分别为0.881及0.867、预测颈总动脉损伤的AUC分别为0.830及0.994。结论 基于颈内静脉直径、颈总动脉直径及二者比值可预测植入TIVAD风险。

      Objective To observe the value of internal jugular vein diameter, common carotid artery diameter and their ratio for predicting the risk of implanting totally implantable venous access device (TIVAD). Methods Totally 46 patients with malignant tumors who underwent TIVAD implantation through internal jugular vein were retrospectively enrolled. The internal jugular vein diameter, common carotid artery diameter and their ratio, as well as puncture times, common carotid artery injury and subcutaneous hematoma occurrence were recorded. The correlations of various parameters were analyzed, and the efficacy of internal jugular vein diameter, common carotid artery diameter and their ratio for predicting puncture time and common carotid artery injury were explored. Results The internal jugular vein diameter, internal jugular vein diameter/common carotid artery diameter and subcutaneous hematoma were moderately correlated with puncture times (r=-0.613, -0.594, 0.588, all P<0.05) ,while the former two were weakly correlated with common carotid artery injury (r=0.323, -0.482, both P<0.05), and common carotid artery injury was highly correlated with subcutaneous hematoma (r=0.884, P<0.05). The area under the curve (AUC) of internal jugular vein diameter and internal jugular vein diameter/common carotid artery diameter for predicting the first successful puncture of TIVAD implantation was 0.881 and 0.867, of predicting common carotid artery injury was 0.830 and 0.994, respectively. Conclusion The internal jugular vein diameter, common carotid artery diameter and their ratio could be used to predict the risk of TIVAD implantation.
