      目的 对比头腹部一站式钆塞酸二钠(Gd-EOB-DTPA)增强MR黑血(BB)序列与T2-液体衰减反转恢复(FLAIR)序列用于检出脑转移癌的价值。方法 回顾性纳入37例经病理证实为恶性肿瘤且合并脑转移的患者,均在上腹部Gd-EOB-DTPA增强扫描后于肝胆期等待时间内先后接受头部增强BB及增强T2-FLAIR扫描;比较2种序列的脑转移癌检出率,以及对于2种序列均可检出病灶的诊断信心评分及其客观评价指标。结果 37例共存在102处脑转移癌。增强BB序列对脑转移癌的检出率高于增强T2-FLAIR序列[92.16%(94/102) vs. 83.33%(85/102),P<0.05],且其整体诊断信心评分及对<3 mm及3~10 mm脑转移癌的诊断信心评分均高于、而对比度噪声比及相对对比度均低于增强T2-FLAIR序列(P均<0.05)。结论 头腹部一站式增强MR方案下,BB序列对于脑转移癌的检出率高于T2-FLAIR序列,而T2-FLAIR序列图像客观指标更优。

      Objective To compare the value of black-blood (BB) and T2-fluid attenuated inversion recovery (FLAIR) sequences in head and abdomen one-stop gadolinium-ethoxybenzyl-diethylenetriaminepentaacetic acid (Gd-EOB-DTPA) contrast-enhanced MR for detecting brain metastases. Methods Totally 37 patients with pathologically confirmed malignant tumors complicated with brain metastases were retrospectively enrolled. All patients underwent upper abdominal Gd-EOB-DTPA contrast-enhanced scanning, followed by contrast-enhanced BB and contrast-enhanced T2-FLAIR sequences of brain during the waiting period of hepatobiliary phase. The detection rate of brain metastases was compared between 2 sequences. For lesions detectable on both sequences, diagnostic confidence scores and objective image quality indicators were compared. Results A total of 102 brain metastatic lesions were found in 37 patients. The detection rate for brain metastases using contrast-enhanced BB sequence was higher than that of contrast-enhanced T2-FLAIR sequence (92.16% [94/102] vs. 83.33% [85/102],P<0.05). The overall diagnostic confidence score, as well as the diagnostic confidence scores for brain metastases <3 mm and 3—10 mm of contrast-enhanced BB sequence were all higher, whereas the contras-to-noise ratio and relative contrast of contrast-enhanced BB sequence were all lower than those of contrast-enhanced T2-FLAIR sequence (all P<0.05). Conclusion Under one-stop contrast-enhanced MR protocol of head and abdomen, BB sequence yielded higher detection rate of brain metastases than T2-FLAIR, while T2-FLAIR had better objective imaging indicators.
