      目的 观察超声引导下经皮射频消融(RFA)原位灭活兔胆囊的可行性与安全性。方法 对10只健康新西兰兔行超声引导下经皮RFA灭活胆囊,记录RFA成功率、消融时间及相关并发症;比较消融前、后血清谷丙转氨酶(GPT)、谷草转氨酶(GOT)及白细胞(WBC)水平,以及超声所测胆囊体积。于消融后30天摘取胆囊及邻近肝组织行HE染色,观察组织病理学改变。结果 对10只(10/10,100%)兔均成功消融,无中转开腹;消融时间30~46 s、平均(39.1±5.4)s。消融后24 h内10只兔均出现食欲减退及活动减少,且均于消融后3天内恢复;消融后30天10只兔均存活,未见肠管、胆总管损伤或大出血等并发症。消融前及其后1、7及30天,GPT、GOT及WBC差异均有统计学意义(P均<0.05);消融前及其后7、30天上述指标均低于消融后1天(P均<0.05)。消融前及其后7、30天胆囊体积差异有统计学意义(P<0.05)且依次减小(P均<0.05)。消融后30天原胆囊结构被致密纤维组织取代并见炎性细胞浸润,邻近肝小叶结构完整。结论 超声引导下经皮RFA原位灭活兔胆囊安全、有效。

      Objective To observe the feasibility and safety of ultrasound-guided percutaneous radiofrequency ablation (RFA) for in situ inactivation of gallbladder in rabbits. Methods Ultrasound-guided percutaneous RFA for inactivation of gallbladder was performed in 10 healthy New Zealand rabbits. The success rate of RFA, ablation time and related complications were recorded, and serum glutamic-pyruvic transaminase (GPT), glutamic-oxaloacetic transaminase (GOT) and white blood cell (WBC), as well as gallbladder volume measured with ultrasound before and after ablation were compared. Thirty days after ablation, the gallbladder and adjacent liver tissue were harvested and stained with HE to observe histopathological changes. Results The ablation was successful in all 10 (10/10, 100%) rabbits without conversion to laparotomy, and the ablation time was 30—46 s, with an average of (39.1±5.4)s. Within 24 h after ablation, decreased appetite and activity were observed in all 10 rabbits, and then recovered within 3 days. Within 30 days after ablation, all 10 rabbits survived without any other complications such as intestinal or common bile duct injury, nor massive hemorrhage. Significant differences of GPT, GOT and WBC levels were noticed before and 1, 7 and 30 days after ablation (all P<0.05), and those before ablation and 7 and 30 days after ablation were all lower than 1 day after ablation (all P<0.05). Significant difference of gallbladder volume was found before and 7 and 30 days after ablation (P<0.05), and the volume decreased sequentially before ablation and 7 and 30 days after ablation (all P<0.05). Thirty days after ablation, original gallbladder structures were replaced by dense fibrous tissue and inflammatory cell infiltration, and adjacent liver lobular structures were intact. Conclusion Ultrasound-guided percutaneous RFA for in situ inactivation of gallbladder in rabbits was safe and effective.
